OUR SERVICES AS A NDIS SERVICE PROVIDER
Community Nursing Care
This service allows our team to provide community nursing care and supports to NDIS participants and the general community who require higher levels of care assistance.
Our services are provided by an Enrolled Nurse (EN) or Registered Nurse (RN) in the following care areas:
- Bowel care
- Wound care
- Catheter care
- Medication administration
- Case Management
- General nursing care
We tailor our services to meet the unique health needs of the person we provide care for.
Assistance with daily living activities
This service provides assistance and supervision of personal tasks of day to day living to enable participants to live as autonomously as possible. This mostly consists of support with hygiene, dressing, communications and lifestyle support. In short terms, it is about providing general personal care and support.
This service helps participants receive care and support within a variety of different shared living arrangements such as SIL,STA,MTA,ILO. It is designed to try an allow participants to live as independently and autonomously as possible whilst still being in a safe and supportive environment.
The care provided will normally include all aspects of personal care such as hygiene requirements and other day to day supports that participants require regularly.
Assistance with travel and transport
This service assists participants with travel/transportation to and from destinations and appointments.
Assistance with household tasks & Development of daily care and life skills
This service assists participants to maintain their general area of living including:
- Cleaning
- Gardening
- Basic indoor & outdoor home maintenance
- Laundry/Linen
- Meal preparation
This service assists participants to develop and maintain important life skills such as learning about public transport, resumes, social skills, physical activity skills etc
Participation in community and social activities
This allows providers to assist participants to attend areas in the community and access main stream activities. For example shopping trips, appointments, leisure activities, football games, fishing trips, bowling etc.
Assistance to access and maintain employment
This service is designed to help individuals and/or groups of individuals find jobs, making use of professional networks to locate and communicate with employers. This also includes supporting those to access and maintain higher education. This may include helping people set goals and plans to acheive those goals whilst then supporting them throughout the process of putting the plan into action.
